Many years ago, I had a great lesson in how easy it is for those in charge to rely on denial when making decisions that affect the lives of those who work for them. I was working in the test engineering department of a prominent removable hard drive company that used Bernoulli technology. Having been very successful with their previous product, they had just released a new 5 1/4" format that provided 20 Megabytes of storage (a lot in the 1980s) but as luck would have it, drives sold to the public were failing at about a 90% rate and generally within 3 weeks.

The Irvine Public Schools Foundation (IPSF) is responsible for managing the after school program for all 26 schools in the district. Using their website, volunteers at each school enter class schedules, enroll children in the classes and assign teachers to these classes.

There was a custom web development company working on several projects in 2008. In fact, growth had been phenomenal and they were busier than ever, so busy in fact they were hiring at a rapid pace. Every project had a series of milestones and on approval by the client for accomplishing the defined tasks, payment was received for the work completed thus far. At the end, the developer would complete a Beta which was the assertion that the project was finished. The client would review the web site and if they found no problems, would pay for final and the site would be made live.

A friend of mine formed a non-profit organization. Over 25+ years, the organization did a lot of good in the business community and the cities it worked with. While focused on the organization's primary purpose, my friend didn't notice the majority of board members had changed. The majority were now employees of the cities they served, not the businesses. The bylaws started to change, activities started to change, membership started to fall off and because the majority of the board members didn't understand how to raise funds, fundraising all but stopped.

I got a call from an attorney. Her staff of seven had lost all access to her client case files and their shared calendar. they had missed court dates, missed client meetings and been sanctioned by the court at least 4 times in a 6 week period.

At the end of a long day, a farmer was sitting on his porch watching his sheep graze as the sun settled in the distance.
